CARE OF MEMORY AT HOME PROGRAM

For dementia patients, a disease which is more and more common around us as life expectancy increases, it is very important to extend their stay at home, thus avoiding their institutionalization.

With a donation from the TIMA Charitable Foundation, NGO FRODIZO implemented the “Care of memory at home” program from February 2015 to December 2016.

The first stage of the donation involved the creation of the “Care of memory at home” pilot project aimed at the physical, mental and psychosocial support of patients with dementia who live in Patras. At its first phase this program targeted elderly people who were diagnosed with first and second stage dementia and their caregivers, through regular visits of a three-member group consisting of a social worker, a psychologist and a physician. In the second phase the actions of the Program were broadened and the existing team was completed by a physiotherapist and a nurse. The services offered were: mental enhancement exercises, psychological support to the elderly and their caregivers, nursing care and physical strength exercises combined with specialized nutritional advice. Also, through home visits, emphasis was placed on training the caregivers, in order to improve patient care and mobility. Through this Program, caregivers were further strengthened through demo education seminars and monthly group meetings that offered them the opportunity to share their concerns and support each other. The success of the Program lies in both the holistic approach of intervention and the personalized design of services tailored to the specific needs of the caregivers. It is worth noting that the Program covered a significant gap in service provision for patients with dementia in Patras, the third largest city in Greece.

In the framework of the project, 70 elderly patients with diagnosed dementia participated, while 64 family carergivers attended educational activities to inform them about care issues. Over the course of the project, more than three thousand (3.000) visits were made by the professionals of our Association to family homes of dementia patients in Patras.
